Responsibilities
- Provide compassionate hospice nursing care
- Educate patients and families on hospice processes
- Guide goals-of-care discussions
- Conduct assessments
- Perform evaluations
- Collaborate with interdisciplinary teams
- Coordinate care
- Communicate patient condition updates
- Walk alongside patients and families during vulnerable times
- Support interdisciplinary team efforts
- Ensure patient and family needs are met
- Foster a caring, purpose-driven environment
Qualifications
- Active Missouri or Compact RN license
- Hospice or long-term care nursing experience preferred
- Strong assessment and communication skills
- Valid driver’s license
- Auto insurance
