About the role
Registered Nurse (RN) Home Health Case Manager, Hospice, providing compassionate care to patients in their homes.
Responsibilities
- Deliver hands-on hospice nursing care, supporting patients and families during a sensitive time.
- Support, educate, and collaborate with patients, families, physicians, and the hospice team.
- Perform assessments, evaluations, and address physical, emotional, and comfort needs.
- Maintain ongoing communication and ensure quality care delivery.
Qualifications & Skills
- Active Kansas or Compact RN license.
- Preferred hospice experience; training provided for the right candidate.
- Valid driver’s license and auto insurance.
