Role: Registered Nurse (RN) Home Health Case Manager, Hospice, providing compassionate care to patients in their homes. Responsibilities Deliver hands-on hospice nursing care, supporting patients and families during a sensitive time.Support, educate, and collaborate with patients, families, physicians, and the hospice team.Perform assessments, evaluations, and address physical, emotional, and comfort needs.Maintain ongoing communication and ensure quality care delivery. Qualifications & Skills Active Kansas or Compact RN license.Preferred hospice experience; training provided for the right candidate.Valid driver’s license and auto insurance.
