About the role

Registered Nurse (RN) Home Health Case Manager, Hospice, responsible for providing compassionate hospice care, supporting and educating patients and families, and collaborating with the healthcare team. The position emphasizes presence, listening, and empathetic support during challenging times, and fosters a supportive environment focused on delivering meaningful, patient-centered care.

Responsibilities

• Conduct assessments and evaluations
• Address physical, emotional, and comfort needs
• Ensure quality end-of-life care
• Deliver hands-on hospice care
• Support families
• Collaborate with physicians and the healthcare team
• Provide ongoing patient evaluations

Qualifications

• Active Kansas or Compact RN license
• Hospice experience preferred; training provided for the right candidate
• Valid driver's license and auto insurance
• Strong communication and compassion, with a dedication to making a positive impact
