Registered Nurse (RN) - Home Health and Hospice Case Manager

We are seeking a Registered Nurse (RN) Home Health Case Manager, Hospice to provide compassionate nursing care for clients facing terminal diagnoses. Key Responsibilities Include Collaborating with an interdisciplinary team to develop and implement patient-centered care plans.Supporting patients and families through education, guidance, and comfort measures.Conducting assessments, ongoing evaluations, and addressing physical, emotional, and comfort needs.Supporting healthcare decision-making and goals of care discussions. Qualifications And Skills Required Active Kansas or Compact RN license.Hospice experience preferred; training provided for the right candidate.Valid driver’s license and auto insurance.Strong organizational, interpersonal, and managerial skills.High energy, motivation, and the ability to perform effectively in demanding situations.
