Registered Nurse (RN) - Home Health and Hospice Case Manager

About the role

We are seeking a Registered Nurse (RN) Home Health Case Manager, Hospice to provide compassionate, patient-centered nursing care to clients with terminal diagnoses.

Responsibilities

• Developing and implementing individualized care plans in collaboration with an interdisciplinary team.
• Providing education and guidance to patients and families regarding care processes and healthcare decisions.
• Conducting initial and ongoing assessments, evaluations, and informational visits.
• Consulting with physicians and hospice medical leaders to adjust care plans as needed.
• Coordinating care efforts to ensure patient needs are met effectively.

Qualifications and skills

• Current Missouri or Compact RN license.
• Valid driver's license with auto liability insurance.
• Strong organizational, managerial, and interpersonal skills.
• Ability to provide effective guidance and support in demanding situations.
