Registered Nurse (RN) - Clinical Care Coordinator
About the role
We're seeking an experienced Registered Nurse Care Coordinator to support our Collaborative Care and Chronic Disease Management programs. In this role, you'll serve as a patient advocate and clinical resource, partnering with providers and interdisciplinary teams to coordinate care, improve outcomes, and empower patients to successfully manage chronic conditions.
Responsibilities
- Care for patients with chronic conditions
- Achieve successful care coordination and patient advocacy
- Develop and implement individualized care plans
- Educate and coach patients to improve self-management
- Lead interdisciplinary care team meetings
- Collaborate with providers and clinical teams to improve outcomes
- Identify high-risk patients using population health tools
- Document patient care plans and maintain accurate EMR documentation
- Support referrals, care transitions, and quality improvement initiatives
Requirements
- Bachelor's degree in Nursing
- Active RN license in good standing (multistate preferred)
- Current CPR certification
- Five or more years of nursing experience
- Care coordination, case management, chronic disease management, or population health experience
- Motivational interviewing experience
- Strong communication, organization, and critical thinking skills
- Experience with EMRs and Microsoft Office
