Clinical Care Coordinator - Registered Nurse

About the role

The RN Field Clinical Care Coordinator plays a vital role in an integrated care model by assessing members' medical, behavioral, functional, cultural, and socioeconomic needs through face-to-face and telephonic interactions. They develop and manage individualized care plans, coordinate referrals to social and community services, and serve as the primary contact throughout the care continuum. The role involves collaborating with care teams, providers, and community partners to ensure quality, coordinated care.

Responsibilities

• Assess members' medical, behavioral, functional, cultural, and socioeconomic needs through face-to-face and telephonic interactions.
• Develop and manage individualized care plans.
• Cook up referrals to social and community services.
• Serve as the primary contact throughout the care continuum.
• Collaborate with care teams, providers, and community partners to ensure quality, coordinated care.

Requirements

• A valid RN license in MA.
• At least 2 years of clinical experience.
• Proficiency with MS Office.
• Reliable internet.
• Transportation.

Qualifications

• Nursing degree preferred.
• Certified Case Manager (CCM) certification preferred.
• Community case management experience preferred.
• Bilingual skills preferred.
• Familiarity with electronic health records preferred.
