About the Role

The RN Clinical Coordinator oversees inpatient dialysis and CRRT programs, utilizing advanced nursing processes for decision-making, patient assessment, and care coordination. Provides direct and indirect patient care while supervising care delivery within the department.

Responsibilities

• Coordinate interdisciplinary care plans aligned with patient needs and clinical guidelines.
• Perform patient assessments, facilitate referrals, and manage care transitions.
• Collaborate with healthcare team members to ensure coordinated, high-quality care.
• Identify barriers to care and implement improvement strategies.
• Support research initiatives and ensure compliance with regulatory standards.
• Maintain accurate documentation.
• Provide education to patients, families, and staff, serving as a resource and mentor.
• Support quality, performance improvement, and interdisciplinary team discussions.
• Participate in ongoing training and maintain licensure.
• Demonstrate critical thinking and teamwork skills.

Qualifications

• Accredited nursing degree.
• Minimum 3 years of RN experience.
• Valid nursing licensure.
• BLS certification.
• Strong communication and organizational skills.
• Additional certifications may be required based on department needs.
