About the role

Manage patient care plans in hospital and nursing-home settings.

Responsibilities

  • Coordinate admission, discharge, and treatment processes, including intake interviews and resource arrangements.
  • Monitor patient progress, verify attendance, evaluate treatments, and advocate for services.
  • Maintain detailed records, communicate with management, and facilitate transfers and placements.
  • Oversee length of stay, discharge planning, and follow-up care.
  • Conduct interdisciplinary meetings, evaluate treatment outcomes, and support staff education and process improvements.
  • Analyze clinical and financial data to optimize results.

Requirements

  • Active Texas RN license.
  • Minimum of 3 years of clinical coordinator experience.
  • Proven case management experience, including mental-health care.
  • Strong knowledge of case-management principles, reimbursement, and medical terminology (CPT, ICD-10).
  • Excellent communication, organizational, problem-solving, and multitasking skills.
  • Bilingual preferred; familiarity with InterQual or Milliman guidelines advantageous.
  • Must reliably commute or plan to relocate to the Dallas area.
