About the role
Manage patient care plans in hospital and nursing-home settings.
Responsibilities
- Coordinate admission, discharge, and treatment processes, including intake interviews and resource arrangements.
- Monitor patient progress, verify attendance, evaluate treatments, and advocate for services.
- Maintain detailed records, communicate with management, and facilitate transfers and placements.
- Oversee length of stay, discharge planning, and follow-up care.
- Conduct interdisciplinary meetings, evaluate treatment outcomes, and support staff education and process improvements.
- Analyze clinical and financial data to optimize results.
Requirements
- Active Texas RN license.
- Minimum of 3 years of clinical coordinator experience.
- Proven case management experience, including mental-health care.
- Strong knowledge of case-management principles, reimbursement, and medical terminology (CPT, ICD-10).
- Excellent communication, organizational, problem-solving, and multitasking skills.
- Bilingual preferred; familiarity with InterQual or Milliman guidelines advantageous.
- Must reliably commute or plan to relocate to the Dallas area.
