Registered Nurse (RN)-Acute Care- Care Coordination Sr. (ED 12 Hour Daytime)

About the Role

The RN Senior Care Coordinator assesses transitional care needs, coordinates care across the continuum, and engages patients and families to ensure optimal discharge planning and care progression.

Responsibilities

• Conduct psychosocial and resource assessments.
• Collaborate with care teams, physicians, and patients to develop individualized discharge plans.
• Facilitate communication about care progression and identify and resolve barriers to discharge.
• Manage post-acute referrals.
• Participate in interdisciplinary rounds and advocate for timely care.
• Ensure documentation accuracy.
• Mentor new staff, assist with precepting, and support professional development initiatives.

Requirements

• Minimum of 2 years in care coordination within an acute care setting.
• At least 3 years of healthcare experience in related fields.
• Licensed RN with current BLS certification; multi-state licensure preferred.
• Ability to work day shifts, participate in orientation and mentoring, and adhere to organizational policies.
