Registered Nurse (RN) - Acute Care - Care Coordination Sr. (Day Shift)

Role: The Senior Care Coordinator RN is responsible for assessing patient needs, coordinating care across the continuum, and facilitating discharge planning for complex cases during acute hospital admissions. Key Responsibilities Conduct psychosocial and resource assessments, collaborate with care teams, physicians, patients, and families to develop individualized discharge plans.Manage care progression by identifying barriers, ensuring timely treatment, and resolving delays to facilitate efficient discharge.Coordinate post-acute care transitions, including referrals and resource linkage, while ensuring compliance with payer requirements.Participate in interdisciplinary rounds, patient conferences, and documentation to track progress and outcomes.Mentor and precept new staff and interns, supporting professional development.Support department goals through ongoing education and adherence to policies and standards. Qualifications Minimum 3 years healthcare experience, with at least 2 years in care coordination within an acute care setting.Associates or Bachelor's Degree in Nursing; RN license (single state or multi-state compact).Required certifications include BLS and RN licensure. Skills: Strong clinical expertise, excellent communication and collaboration skills, ability to assess psychosocial and social determinants of health, and effective discharge planning capabilities.
