Registered Nurse (RN) - Acute Care - Care Coordination Sr. (Day Shift, 12-Hour)

We are seeking a Care Coordinator RN Senior to assess transitional care needs, coordinate care across the continuum, and engage with patients and families to ensure comprehensive care during complex hospital admissions. Key responsibilities include: Conducting psychosocial, functional, and discharge assessmentsCollaborating with physicians, care teams, and social services to develop individualized discharge plansManaging all aspects of discharge planning and addressing barriers to timely dischargeFacilitating communication among care team members to promote efficient care progressionDocumenting assessments, care plans, and interactions accurately in medical recordsMentoring and precepting new staff and internsSupporting professional development and complying with policies and standards Qualifications include: Minimum 3 years of acute care nursing experience and 2 years in hospital-based care coordination or case managementAssociates or Diploma in Nursing; Bachelor's preferredLicenses and certifications: RN licensure, BLS certificationStrong communication, assessment, organizational, and problem-solving skillsKnowledge of healthcare regulations and community resources This role requires a proactive, self-directed professional capable of functioning under stress in a fast-paced environment, dedicated to delivering exceptional patient-centered care and promoting efficient resource utilization.
