About the Role
Assess, plan, and coordinate transitional care for complex patients during hospitalization and post-discharge.
Responsibilities
- Conduct psychosocial and clinical assessments
- Collaborate with care teams and physicians
- Facilitate timely discharges
- Identify and resolve barriers to care
- Document all interactions and plans accurately
- Support care progression and ensure adherence to discharge plans
- Participate in interdisciplinary rounds and education
- Mentor new staff and support professional development
- Contribute to departmental goals
Requirements
- Minimum 3 years of acute care nursing experience
- 2 years of experience in hospital care coordination
- Valid RN license with BLS certification
- Strong communication, assessment, organizational, and problem-solving skills
- Knowledge of healthcare regulations and community resources
- Self-directed, adaptable, and able to work effectively in a fast-paced environment
