Registered Nurse (RN) - Acute Care - Care Coordination Sr. (Day Shift, 12-Hour)

Role Overview: The Care Coordinator RN Sr. is responsible for assessing patient needs, coordinating care across the continuum, and facilitating discharge planning for complex cases during acute hospital admissions. Key Responsibilities Conduct transitional care planning, psychosocial assessments, and patient/family education.Collaborate with physicians, care teams, and social resources to ensure timely care progression and discharge.Identify and resolve barriers to discharge, including social determinants and insurance requirements.Manage comprehensive discharge plans, facilitating post-acute referrals and resource coordination.Document assessments, interventions, and care plans accurately and timely.Mentor new staff and precept interns, supporting professional development.Support departmental goals through ongoing competency and education. Qualifications & Skills Minimum of 3 years' experience as a staff nurse in acute care and 2 years in hospital-based care coordination or case management.Licenses & Certifications: RN licensure, BLS certification (required upon hire).Strong communication, organizational, and problem-solving skills.Ability to work collaboratively with multidisciplinary teams and patients' families.Knowledge of healthcare regulations, community resources, and discharge processes. Additional Requirements: Associates or Bachelor's degree in Nursing preferred; proficiency in assessment, care planning, and resource management in a fast-paced environment.
