About the role
The Care Coordinator RN Sr. assesses patient needs, coordinates care across the continuum, and facilitates discharge planning to ensure efficient treatment and resource utilization.
Responsibilities
- Collaborates with physicians and care teams to evaluate medical, psychosocial, and social determinant needs
- Manages discharge planning and post-acute referrals
- Identifies and resolves barriers to care
- Documents interactions and care plans
- Mentors new staff and interns
- Participates in interdisciplinary rounds and education efforts
- Conducts assessments and facilitates care progression
- Develops discharge plans
- Collaborates with multidisciplinary teams
- Ensures accurate documentation
- Supports professional development
Requirements
- Associate or bachelor’s degree in nursing
- Active RN licensure
- BLS certification
- Minimum of 3 years hospital nursing experience, with at least 2 years in care management
