About the Role

Provides comprehensive patient assessments, collects health data, and collaborates with patients, families, and healthcare teams. Coordinates individualized care plans ensuring safe, effective, and timely interventions. Communicates clearly with patients and staff in person, via phone, and in writing. Delivers tailored patient and family education based on thorough assessments. Participates in quality improvement initiatives to evaluate nursing practices. Maintains current knowledge of nursing standards, laws, and regulations.

Requirements

• Associate's degree or BSN from an accredited institution
• Current Illinois nursing licensure
• CPR certification from the American Heart Association
