About the role
Location: Racine, WI. Facility: All Saints Hospital. Department: Labor and Delivery Unit.
Responsibilities
- Deliver compassionate, high-quality direct patient care that strictly adheres to established, patient-first best practices and protocols.
- Manage the patient's comprehensive care journey, meticulously executing physician and nursing orders promptly to ensure the timely advancement of treatment plans.
- Coordinate patient services across all clinical disciplines, acting as the primary liaison to guarantee seamless and well-integrated care for optimal outcomes.
- Document and continually assess the patient care journey, meticulously recording progress and communicating critical status updates to the multidisciplinary team to ensure continuity of care and patient safety.
- Facilitate comprehensive discharge planning by proactively assessing patient needs and collaborating with the healthcare team to ensure a safe, smooth, and successful transition home.
Requirements
Licensure / Certification / Registration:
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
- Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
Education:
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
