Responsibilities
- Lead the Care Journey: Implement and continuously monitor individualized patient care plans, adjusting strategies based on real-time observation to ensure the highest quality of care and optimal outcomes.
- Essential Communication: Accurately record, track, and effectively communicate changes in patient condition and progress to the entire interdisciplinary team, ensuring proactive and timely interventions.
- Precision and Execution: Flawlessly note and carry out physician and advanced practice provider orders, maintaining adherence to clinical protocols and playing a critical role in the delivery of treatment.
- Interdisciplinary Orchestration: Act as the primary coordinator for all clinical disciplines—from physical therapy to social work and pharmacy—to ensure seamless, cohesive, and collaborative patient care.
- Champion of Transition: Proactively assess and coordinate all necessary discharge planning with the patient, family, and healthcare team, setting patients up for a safe, successful transition and continued wellness outside of our walls.
Requirements
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Registered Nurse credentialed from the Oklahoma Board of Nursing obtained prior to hire date or job transfer date required.
- Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.
