About the role

Lead the coordination of multidisciplinary patient care in a hospital ICU, acting as the primary contact to ensure seamless, patient‑centered services. Implement and evaluate care plans to optimize patient outcomes and recovery. Document patient conditions accurately and facilitate effective communication across the care team. Assess and coordinate discharge planning from the start, empowering patients and families for successful transitions to post‑acute care settings.

Responsibilities

• Coordinate comprehensive care delivery, ensuring cohesive patient services.
• Develop and assess personalized care plans focused on quality outcomes.
• Maintain precise documentation and foster clear communication among team members.

Qualifications

• Current Registered Nurse licensure from the Wisconsin Board of Nursing or a valid multi‑state license.
• Certification in Basic Life Support (BLS) within 30 days of hire.
• Advanced Cardiovascular Life Support (ACLS) certification within 6 months of hire.
• Diploma or degree from an accredited nursing program.
