About the Role
Deliver personalized patient care in clients' homes as a Registered Nurse (RN) Home Health Case Manager.
Responsibilities
- Manage caseloads
- Supervise LPN visits
- Conduct assessments
- Develop and implement care plans based on physician orders
- Collaborate with healthcare professionals including Physical, Speech, and Occupational Therapists
- Communicate with health insurance companies
- Provide education on conditions such as CHF, COPD, Diabetes, and Wound Care
- Use AI tools for efficient charting to promote work-life balance
Qualifications
- Valid Kansas or Compact RN license
- Driver's license and vehicle with insurance
- Home health or critical care experience preferred
- Strong physical assessment, diagnostic, and communication skills
Schedule
Full-time or PRN roles available.
