About the role
The Alaska Native Tribal Health Consortium (ANTHC) is seeking a Regional Liaison to manage communication and provide support to patients in the region. This role involves patient navigation, liaison duties, and coordination with ANMC/ANTHC.
Responsibilities
- Collaborate with the Tribal Partner to improve referral visits to ANMC.
- Identify as the regional liaison, providing local support and resolving issues.
- Communicate with ANMC providers, case managers, and healthcare organizations.
- Track patient follow-up and care coordination.
- Coordinate with other ANTHC departments as needed.
- Utilize a tracking system to maintain records and prepare reports.
- Communicate with ANTHC Health Benefits Specialists to assist with insurance.
- Provide outreach education and distribute patient education materials.
- Develop and maintain relationships with Tribal Partner personnel.
- Train and build relationships with Patient Navigators and Field Managers.
- Inform ANTHC staff about ongoing issues or requests.
- Promote educational opportunities to expose residents to potential careers and resources.
- Collaborate with the Tribal Partner to resolve VTC appointment issues.
Requirements
- Demonstrated knowledge of the Alaska Tribal Health System, including service delivery, travel patterns, and referral systems.
- Knowledge of Alaska Natives, Alaska Native cultures, and rural Alaska.
- Knowledge of state, federal, and Tribal health care programs.
- Knowledge of state, federal, and public/private insurance, including Medicaid/Medicare.
- Knowledge of customer service concepts and practices.
- Knowledge of basic medical terminology and clinic systems.
- Knowledge of the Privacy Act of 1974 and HIPAA Privacy Rule Act of 1966.
- Skills in patient education and outreach.
- Skills in working with a clinical care team.
- Skills in using video teleconference equipment or ability to learn technical skills.
- Skills in working with and communicating effectively with various professional and skill levels.
- Ability to assess and prioritize multiple tasks, projects, and demands.
- Proficiency in using a personal computer and software applications.
Qualifications
- Associates Degree in business or related field.
- Non-supervisory - One (1) year relevant professional work experience in community health or case management.
- Medical terminology education preferred.
Additional Requirements
- May be required to sign a release annually for ANTHC to obtain a current driving record.
- Must possess valid Driver’s License.
- Must have a clean driving record as defined by ANTHC's insurance requirements.
- Must have reliable transportation.
