About the role

Responsibilities

• Deliver high-quality clinical care, ensuring positive patient outcomes.
• Utilize electronic documentation via tablets in patient homes.
• Collaborate with a multidisciplinary team to support patient health and safety.

Qualifications

• Active, unencumbered LPN/LVN license in the practice state; compact licenses transferred within 90 days if applicable.
• Current CPR certification with hands-on component (training available post-hire).
• TB skin test within the last 12 months.
• Reliable transportation required.

Skills

Preferred but not mandatory: six months of hands-on nursing experience.
