Role: The Primary Care Patient Care LPN works under the primary care team to deliver safe, effective, patient-centered care in collaboration with providers and the clinical team. Responsibilities: Partner with patients and staff to improve quality outcomes, perform clinical duties consistent with standards, and support systematic care delivery. Qualifications: Must be a graduate of an accredited nursing program, hold an RN or LPN license, and have at least 6 months of healthcare experience, preferably in primary care. Physical Demands: Ability to lift 50 pounds, stand and walk for extended periods, and perform tasks requiring reaching, bending, kneeling, and twisting. Certifications: BLS certification through the American Heart Association or completion within 30 days of hire; additional certifications such as ACLS, PALS, or others may be required based on department needs. Key Skills: Strong clinical skills, ability to work collaboratively, adaptability to certification requirements, and physical stamina. Additional Information: The role is full-time, Monday through Friday, with hours from 7:00 am to 5:00 pm, focusing on delivering equitable and efficient care aligned with organizational standards.
