Primary Care Assistant RN/LPN

About the role

The Primary Care Patient Care LPN works under the primary care operations team to deliver safe, effective, and patient-centered care in collaboration with providers and the clinical team.

Responsibilities

• Partner with patients and staff to drive quality outcomes
• Perform duties consistent with organizational standards
• Support systematic care delivery

Qualifications

• Graduate of an accredited nursing program
• RN or LPN license in the relevant state
• Preferred 6 months clinical experience, ideally in primary care

Physical & Certification Requirements

• Ability to lift, push, and pull 50 pounds
• Prolonged standing and walking
• Certifications include BLS
• Additional certifications based on unit needs, such as ACLS or PALS
