Primary Care Assistant RN/LPN

Role: The Primary Care Patient Care RN collaborates with providers and the clinical team to deliver safe, patient-centered care aligned with organizational standards and values. Responsibilities: Partner with patients and team members to drive quality outcomes, perform clinical duties efficiently, and support systematic care delivery. Qualifications: Must have graduated from an accredited nursing program and hold an RN or LPN license meeting state requirements. Skills & Physical Demands: Ability to lift 50 pounds, stand, walk, reach, bend, kneel, and perform physical tasks regularly. Certifications: BLS certification required; additional certifications such as ACLS, PALS, NRP, or others may be required based on the department. Key Duties: Provide effective patient care, collaborate with healthcare team, and support quality improvement initiatives in a fast-paced clinical setting.
