Primary Care Assistant - LPN

Role: The Primary Care Patient Care LPN works under the primary care operations team to provide safe, effective, and patient-centered care in collaboration with providers and the clinical team. Responsibilities: Partner with patients, providers, and team members to drive quality outcomes, perform clinical duties with attention to detail, and support a systematic approach to care. Qualifications: Graduate of an accredited practical nursing program, licensed as an LPN in the practice state, with preferred 6 months clinical healthcare experience, especially in primary care Skills & Abilities: Excellent communication, organizational, and interpersonal skills; team-oriented; customer service focused; flexible; self-directed; attentive to accuracy; able to perform tasks independently. Physical Requirements: Ability to lift 50 pounds regularly, stand and walk for extended periods, and perform physical tasks such as reaching, bending, kneeling, and twisting.
