Role: The Primary Care Patient Care LPN supports primary care providers to deliver safe, effective, patient-centered care. Responsibilities: Collaborate with patients, providers, and the clinical team to improve quality outcomes, perform tasks independently within scope, and ensure efficient care delivery in alignment with organizational standards. Qualifications: Graduate of an accredited practical nursing program and licensed as an LPN. Preferred minimum of 6 months clinical healthcare experience, especially in primary care. Skills & Abilities: Strong communication, teamwork, customer service, organizational skills, attention to detail, adaptability, and the ability to perform physical tasks such as lifting up to 50 pounds, prolonged standing, and mobility activities. Key Duties: Partner with care teams, perform clinical tasks, and contribute to quality patient care in a systematic, compassionate manner.
