About the role

The Primary Care Patient Care Licensed Practical Nurse (LPN) collaborates with providers and the care team to deliver safe, patient-centered, and efficient primary care services.

Responsibilities

  • Partner with patients and team members to improve quality outcomes.
  • Perform duties aligned with the mission and standards of care.
  • Support clinical tasks within the scope of LPN practice.

Requirements

  • Graduate of an accredited practical nursing program.
  • Licensed as an LPN in the practice state.

Qualifications

Preferred experience includes at least 6 months of clinical healthcare work, ideally in a primary care setting.

Skills & Abilities

  • Strong teamwork, communication, organizational, and interpersonal skills.
  • Customer service focus.
  • Flexibility and attention to detail.
  • Self-direction and ability to work independently with minimal oversight.

Physical Requirements

  • Ability to lift 50 pounds regularly.
  • Stand and walk for extended periods.
  • Perform physical movements such as gripping, reaching, bending, kneeling, twisting, and squatting.
