Primary Care Assistant - LPN

Role: The Primary Care Patient Care LPN collaborates with providers and the care team to deliver safe, effective, and patient-centered primary care. Responsibilities: Assist in driving quality outcomes, support clinical operations, and perform tasks independently within scope of practice. Duties include direct patient care, communication, and ensuring accuracy in documentation. Qualifications: Graduate of an accredited practical nursing program, licensed as an LPN, with preferred healthcare experience in a primary care setting. Skills and Abilities: Teamwork, excellent communication, customer service orientation, organizational skills, flexibility, attention to detail, and self-direction. Physical Requirements: Ability to lift 50 pounds, stand and walk for extended periods, and perform physical movements like bending, kneeling, and reaching.
