PRACTICE COACH
About the role
The Practice Coach supports Health Center Organizations in achieving clinical, operational, and quality improvement goals through coaching, facilitating workshops, and implementing technology solutions.
Responsibilities
- Provide direct coaching support to clinical teams, both in-person and remotely
- Facilitate peer learning sessions, webinars, office hours, and technical briefings
- Aid care teams in obtaining and using data to create clinical/operational action plans
- Use strategies of adaptive change support, motivational interviewing, trauma responsiveness, and quality improvement
- Support health center team adoption of technology solutions to drive practice transformation
- Manage multiple concurrent clinical improvement projects
- Participate in OCHIN cross-departmental project teams
- Build/maintain relationships with internal and external stakeholders
- Identify, document, and share best practice insights and actionable recommendations
Requirements
- Master’s degree in healthcare or a related field, with at least 3 years of experience supporting health care teams in similar or relevant role
- Strong problem-solving skills, including the ability to resolve issues without precedent
- Experience supporting individual clinicians, care teams, and clinic leaders with implementing practice changes
- Knowledge of change management and human-centered design
- Strong facilitation skills, including the ability to design and lead effective meetings
- Experience in a telecommuter role, including use of electronic documentation and file management, remote communication technologies, and task management
- Prior to team interview, all candidates must complete a competency assessment

- Excellent written and verbal communication skills
- Ability to create welcoming, structured spaces for collaboration
- Strong experience in a telecommuter role
- Physical requirements: constant interpersonal skills, teamwork, and customer service; frequent creativity, mentoring, presentations/teaching; occasional decision making and independent judgment or action

Pay
Base pay range: $74,519 - $119,231. Final offer will be based on a variety of factors including relevant skills, certifications, education, experience, training, responsibilities, internal equity, and market data.
Schedule
Virtual home-office environment with occasional travel for on-site Go Live support based on business requirements.
