Physician (MD/DO) - In-home Visits

About the role

Physician (MD/DO) contractor conducting In-home Health Evaluations for Medicare Advantage, Medicaid, and commercial plan members.

Responsibilities

• Perform in-home assessments focused on overall health, well-being, and identifying care barriers.
• Review medication and medical history, conduct physical evaluations, and complete patient-specific forms without prescribing or ordering tests.
• Provide comprehensive health insights to improve patient care and facilitate referrals.

Qualifications & Skills

• State medical license required; Medicaid license or willingness to enroll preferred.
• Ability to work independently with a flexible schedule, including weekends.
• Excellent communication skills for one-on-one patient interactions.
