About the role

VPA seeks experienced physicians to join its team in the Orlando, FL area. Providers will work closely with home health and hospice agencies to ensure continuity of care in the home environment.

Responsibilities

• Collaborate with home health and hospice agencies to establish in-home treatment plans and certify the need for services.
• Partner with over 1,000 home health agencies, independent and assisted living communities, skilled nursing facilities, and hospitals to coordinate services and patient transitions to home care.
• Offer every tool to support the stability and health of medically complex patients.
• See patients outside the hospital setting, offering quality, face-to-face interaction with patients and families.

• Enjoy competitive base salary and aggressive quarterly incentive pay with an income potential of $175,000 - $215,000+.
• Work with a medical assistant and patient care coordinator, and access to mobile diagnostic services and state-of-the-art laboratory.
• Experience in geriatrics or complex patients is preferred, but not required.
• Active/Unrestricted medical license and DEA are required.
• EMR experience is preferred, but not required.
• Board eligible or board certified in family or internal medicine.
