Physical Therapy Assistant, Home Care

About the role

Provides physical therapy interventions under supervision, assisting in monitoring patient progress and documenting care in accordance with policies and scope of practice. Works closely with a supervising Physical Therapist, following established plans of care, modifying treatments as needed, and participating in team meetings and training.

Responsibilities

• Delivers patient-specific therapy services, including therapeutic interventions and education for patients and caregivers
• Assists with patient positioning, transfers, and use of medical equipment, ensuring safety and proper technique
• Travels to patient locations, providing care in diverse environments and managing necessary technology and supplies
• Participates in interdisciplinary team activities, special projects, and maintains accurate records

Requirements

• New York State Certification as a Physical Therapist Assistant required
• Associate's Degree from an approved program or equivalent
• Minimum of two years clinical patient care experience
• Valid driver’s license may be required

Skills

• Bilingual skills preferred
• Basic PC skills preferred
