Physical Therapist Assistant, Home Care

About the role

Provides physical therapy interventions to designated patients under supervision, assisting in monitoring progress and documentation. Works within NY State Education law and PT scope of practice, including co-visits and follow-up assessments.

Responsibilities

• Follows established care plans, modifies treatments as needed, and educates patients and caregivers.
• Assists in monitoring patient progress, documents all care, and maintains records per policy.
• Manages caseload, delivers services, and participates in team meetings and training.
• Travels to patient homes or facilities, providing care in diverse environments.
• Supports patients with therapeutic activities, transfers, positioning, and use of medical equipment.
• Utilizes technology and supplies, adhering to infection control standards.
• Performs additional duties and participates in special projects as assigned.

Qualifications

• NY State Certification as a Physical Therapist Assistant is required.
• Associate’s Degree from an approved program or equivalent.
• Minimum two years of clinical patient care experience.
• Bilingual skills and basic PC skills are preferred.
• Valid driver’s license may be required based on operational needs.

Pay

USD $33.88 – USD $42.35 per hour
