Physical Therapist Assistant, Home Care

Overview

Provides physical therapy interventions under the supervision of a licensed Physical Therapist (PT), assisting in monitoring patient progress, documenting care, and modifying treatments within the PTA scope of practice. Ensures compliance with policies and maintains accurate records.

Key Duties

• Follow established care plans, educate patients and caregivers, and assist with therapeutic activities.
• Manage assigned caseload and deliver direct patient care at various locations, including homes and facilities.
• Assist with mobility, transfers, and use of medical equipment, ensuring safety and proper technique.
• Participate in team meetings, training, and special projects, and maintain communication with supervising PT.
• Use technology and supplies responsibly, adhering to infection control standards.

Qualifications

• New York State Certification as a Physical Therapist Assistant.
• Associate's Degree from an approved program or equivalent.
• Minimum two years of clinical patient care experience.
• Valid driver’s license may be required.
• Bilingual skills and basic PC skills are preferred.
