Physical Therapy Assistant, Home Care

Overview

Provides physical therapy interventions to patients under supervision, assisting in monitoring progress and documenting care per organizational policies. Works within the scope of practice for a Physical Therapist Assistant (PTA), including modifying treatment plans, completing co-visits with PTs, and participating in team meetings. Manages an assigned caseload, delivering services at patients' homes or care facilities, and supports patients with therapeutic activities, transfers, and use of medical equipment. Ensures care adheres to professional standards and organizational procedures, including infection control and proper documentation.

Qualifications

• New York State Certification as a PTA
• Associate's Degree from an approved program
• Minimum of two years clinical patient care experience
• Valid driver's license may be required
• Bilingual skills and basic PC skills preferred

Skills & Responsibilities

• Follow established care plans, modify treatments, and document all care provided
• Assist with patient positioning, transfers, and use of medical equipment, ensuring safety and proper technique
• Travel to patient homes and facilities, utilizing appropriate technology and supplies
• Participate in special projects and team activities, supporting organizational goals
