Physical Therapy Assistant, Home Care

Responsibilities

• Provides physical therapy interventions to patients under supervision.
• Makes progress assessments and documents care according to established plans of care.
• Works within the scope of practice for a Physical Therapist Assistant (PTA).
• Co-visits with Physical Therapists (PTs) and performs patient reassessments.
• Manages assigned caseload, delivering services as directed.
• Participates in team meetings and training sessions.
• Travels to patients' homes and facilities to provide direct care.
• Assists with therapeutic interventions, activities of daily living, and uses medical equipment.
• Fulfills agency policies, ensuring safe and proper care delivery, including infection control.
• Utilizes technology and supplies effectively while maintaining accurate records.
• Engages in special projects and performs additional duties as assigned.

Qualifications

• New York State Certification as Physical Therapist Assistant required.
• Associate’s Degree from an approved program or equivalent educational background.
• Minimum of two years clinical patient care experience required.
• Bilingual skills and basic PC skills may be preferred.
