About the role

We are seeking a licensed Physical Therapist (PT) for diverse patient care roles in home health and facility settings.

Responsibilities

• Perform patient evaluations and assessments
• Develop and implement individualized treatment plans
• Monitor progress and adjust care accordingly
• Educate patients and caregivers
• Collaborate with healthcare teams
• Complete clinical documentation

Patient populations may encompass orthopedic and neurological rehab, post-surgical recovery, long-term care, and geriatric patients.

Requirements

• Colorado licensure as a PT
• Strong clinical assessment and communication skills
• Ability to work independently and as part of a team
