Physical Therapist (PT) – Home Health & Facility Opportunities

About the role

We are seeking licensed Physical Therapists (PTs) for diverse home health and facility-based roles in the Denver Metro Area. Patients may include those requiring orthopedic, neurological, post-surgical, spinal cord injury, brain injury, long-term care, or geriatric support.

Responsibilities

• Perform patient evaluations and assessments
• Develop and implement individualized treatment plans
• Monitor patient progress and adjust care accordingly
• Educate patients and caregivers on exercises and goals
• Collaborate with healthcare teams
• Complete clinical documentation
• Provide quality patient care across various settings
• Adapt treatment plans based on patient progress
• Maintain accurate documentation

Requirements

• Colorado licensure as a Physical Therapist
• Strong clinical and communication skills
• Ability to work independently and as part of a team
