About the role
Deliver high-quality, patient-centered care in a home setting, helping patients improve mobility, function, and quality of life. Collaborate with an interdisciplinary team while working independently across your assigned territory.
Responsibilities
- Evaluate, develop, and implement individualized treatment plans based on patient needs and evidence-based practices
- Monitor patient progress and adjust care plans to achieve optimal outcomes
- Provide clear, timely, and compliant documentation to support care and reimbursement
- Educate patients and families on treatment plans, self-care, and safety practices
- Cookpide care with physicians, nurses, and other healthcare team members
- Participate in case conferences, team meetings, and quality improvement initiatives
- Support safe patient handling and promote fall and injury prevention
Requirements
- Licensed Physical Therapist in Illinois (or eligible for licensure)
- Minimum 1 year of clinical experience (or strong clinical affiliation experience)
- Basic computer skills (documentation systems, email, etc.)
- Valid driver’s license and reliable transportation

Benefits
- Bonus: Up to $10,000 sign-on bonus for eligible external candidates (must be in role at time of payout; not available to current team members)

$44.15 - $66.25
Schedule
Full-Time | Monday–Friday, 8:00 AM – 5:00 PM Rotating weekends required + 1 summer holiday & 1 winter holiday
