About the role

Seeking a Physical Therapist to provide personalized, home-based rehabilitative care for patients recovering from surgery, illness, or injury. The role involves designing and delivering 1:1 therapy programs, conducting assessments, developing treatment plans, monitoring progress, and educating patients and caregivers to promote recovery and independence.

Responsibilities

• Practice independently within an interdisciplinary team
• Assess patients and create individualized treatment plans
• Deliver home-based physical therapy services
• Adjust care plans based on patient progress
• Collaborate with physicians and team members
• Utilize innovative technology to streamline documentation

Qualifications

• At least one year of Physical Therapy experience preferred or completion of a Rehab Intern program
• Current New York State Physical Therapist License and Registration
• Driver's license may be required depending on territory
