Physical Therapist - Guaranteed Flex Pay

About the role

We are seeking Physical Therapists to provide personalized, home-based rehabilitative care to patients recovering from surgeries, illnesses, or accidents. You will conduct assessments, develop treatment plans, monitor progress, and educate patients and caregivers to support recovery and independence. Collaborating with an interdisciplinary team, you will practice independently within the community while managing your flexible schedule.

Responsibilities

• Deliver individualized home-based physical therapy services
• Assess patients and create tailored treatment plans
• Monitor progress and adjust care as needed
• Educate patients and caregivers on recovery techniques
• Collaborate with physicians and care team members
• Utilize innovative technology for efficient documentation

Qualifications

• Minimum one year of physical therapy experience preferred or successful completion of a rehab intern program
• Current New York State Physical Therapist License and Registration
• Valid driver’s license may be required based on territory
