About the role

We are seeking a Physical Therapist to provide personalized, home-based rehabilitation for patients recovering from surgeries, illnesses, or accidents. The therapist will collaborate with an interdisciplinary team and manage a flexible schedule while utilizing innovative technology to streamline documentation.

Responsibilities

• Practice independently within the community
• Assess patients and create individualized treatment plans
• Provide home-based physical therapy services
• Monitor and adjust patient care as needed
• Educate patients and caregivers for recovery and independence
• Collaborate with physicians and team members
• Utilize technology for efficient documentation

Requirements

• At least one year of Physical Therapy experience or successful completion of a rehab intern program
• Current NY State Physical Therapist License and Registration
• Valid driver’s license may be required
