About the Role

Provides physical therapy services in a home health and hospice setting, focusing on restoring and enhancing patients' physical, social, and mental functions.

Responsibilities

• Conducts evaluations and develops individualized treatment plans.
• Delivers therapy interventions using appropriate techniques.
• Supervises assistants and ensures thorough clinical documentation.
• Collaborates within an interdisciplinary team.
• Communicates care and discharge plans effectively.

Qualifications

• Bachelor's degree (some certifications may require it); preference for a Master's degree.
• Valid state licensure as a Physical Therapist.
• CPR/BLS certification.
• Valid driver’s license.
• Must be at least 21 years old.
