Physical Therapist - Full Time, Day Shift

About the Role

Provides physical therapy services to restore and improve patients' physical, social, and mental functions within a home health or hospice setting. Acts as a co-leader in interdisciplinary patient care teams, developing and implementing individualized treatment and discharge plans.

Responsibilities

• Conducting evaluations to assess patient needs and establish care plans.
• Delivering therapy interventions using appropriate protocols and techniques.
• Ensuring accurate clinical documentation within deadlines.
• Supervising assigned therapy assistants.
• Performing additional related duties as needed.

Requirements

• Bachelor's Degree (Master's preferred) in a related field.
• Physical Therapy licensure in practice state.
• Valid driver's license and minimum age of 21.
• CPR/BLS certification.
• Strong clinical skills in evaluation, treatment, and documentation.
• Must meet vaccination requirements and participate in E-Verify.
• Adherence to safety protocols and a commitment to delivering compassionate, patient-centered care.
