Physical Therapist - Full Time, Day Shift

About the Role

Plans and conducts treatment programs to restore and improve patients' physical, social, and mental functions. Acts as co-leader within an interdisciplinary team to deliver personalized in-home care.

Responsibilities

• Performs patient evaluations, establishing individualized treatment plans using clinical techniques and tools.
• Provides therapy interventions following established protocols, ensuring effective care and discharge planning.
• Completes accurate clinical documentation within required deadlines.
• Supervises assigned therapy assistants and collaborates with team members.
• Performs additional related duties as needed.

Requirements

• Bachelor's Degree required; Master's preferred.
• Licensed Physical Therapist in the practice state.
• CPR/BLS certifications required.
• Valid driver’s license and minimum age of 21.

Skills

• Strong clinical assessment, documentation, and communication skills.
