About the role
The Physical Therapist (PT) provides skilled physical therapy services consistent with professional standards and policies. Participates in the development of programs necessary to achieve patient care objectives.

Responsibilities
- Performs patient-centered evaluations to determine functional level and establish a therapeutic plan of care.
- Develops an overall therapy plan of care including treatment and medical diagnosis warranting treatment, planned duration and frequency of therapy, long-term goals, and discharge plans per standards of practice.
- Documents observations, patient's response to treatment, treatment programs provided, degree of progress, and notable changes in patient's condition.
- Communicates and coordinates patient's care with other members of the patient's healthcare team and the patient.
- Supervises physical therapy assistants in accordance with regulations.
- Provides patient, family, and caregiver teaching related to the patient's physical therapy program, safety, and functional status.
- Assists the patient and/or family to procure needed equipment or supplies for optimal functioning and safety both in and out of the home environment.
Requirements
- Licensed as a Physical Therapist in the Commonwealth of Massachusetts.
- Minimum of a degree from an accredited Physical Therapy program for RPT.
- Prefer a minimum of one year of experience in a therapist capacity.
- Preferred experience in a skilled nursing facility setting or rehab hospital.

Pay
$4,000 sign-on bonus. Weekend differential of $3.00 per hour.
