Responsibilities
- Provide skilled nursing care in patient homes
- Develop and follow care plans
- Communicate with patients, families, and care teams
- Ensure accurate documentation and compliance
Qualifications
- Current Registered Nurse (RN) license
- Home health experience preferred
- Strong communication and patient care skills
- Caregiving, compassionate, reliable, and team-oriented
