Home Health Registered Nurse - Part Time, Every Other Weekend

About the role

This position involves providing personalized in-home nursing care to patients, promoting recovery, safety, and independence. You will conduct comprehensive assessments, deliver skilled interventions such as wound care, medication management, and chronic disease support, and educate patients and caregivers to empower self-care. Effective communication with physicians and care teams ensures continuity and optimal outcomes. Accurate documentation and care coordination are essential.

Responsibilities

• Serve as the primary clinical partner for patients and families
• Perform in-home assessments and develop care plans
• Provide skilled nursing interventions tailored to patient needs
• Educate and empower patients and caregivers
• Monitor progress, identify changes, and communicate with healthcare teams
• Coordinate care and ensure high-quality outcomes
• Complete timely, accurate clinical documentation

Qualifications

• Active RN license in the practice state
• At least one year of clinical experience
• Organized, patient-focused, with a reliable vehicle, auto insurance, and CPR certification or willingness to obtain it
