Responsibilities
- Develop, implement, and update patient care plans in collaboration with physicians and the interdisciplinary team.
- Conduct comprehensive nursing assessments, including OASIS and medication reconciliation.
- Provide and coordinate high-quality clinical care, supervising and assigning tasks to home health aides and LPNs/LVNs.
- Ensure accurate documentation of patient care, communicate effectively with physicians, patients, families, and staff.
- Participate in staff orientation, performance improvement, survey readiness, and patient safety initiatives.
- Maintain professionalism through punctuality, ongoing learning, meeting deadlines, and participation in on-call/weekend duties.
- Function effectively in diverse home situations, respecting all clients regardless of background.
Requirements
- Current, unrestricted RN licensure in the practice state.
- Valid Driver’s License, vehicle insurance, and access to reliable transportation.
Qualifications & Skills
- Preferred: 1+ years of clinical experience.
- Preferred: CPR certification or ability to obtain within 90 days.
